Skilled Nursing Services With RN Case Management

At Milestone Group Homes, we understand that apart from the challenges that are present with having a developmental and/or intellectual disability, there are also corresponding administrative concerns to deal with. This is why, our group home provides our Skilled Nursing Services along with RN Case Management. This service includes Medication Administration Services and also covers the tasks that an RN Case Manager does such as:

• Assessing new residents
• Updating and revising patient health care plans as necessary
• Educating patients and families about healthcare plans and options
• Making clinical decisions for routine resident care
• Documenting and submitting medical records
